Radical Health brings health care professionals together with members of underserved communities to develop strategies to increase access to and engagement with quality holistic health care and social services. Through an online platform that allows health care professionals to identify and refer clients to community-based organizations that address the social determinants of health, Radical Health addresses health disparities in underserved communities through collaboration with local, grassroots neighborhood resources. Our online platform allows direct service providers, medical professionals and hospitals / organizations to access a network of hundreds of thousands of community resources that provide housing, food pantries, counseling, support groups, job training, employment, and other services. Our team of health care experts combines years of experience in innovative health care technology with practical solutions from the people experiencing health disparities. Through consultation, focus groups, and assessments, we help large health care providers develop strategies to engage and impact underserved communities that actually work.
